On May 4, 2023, a refinery in California experienced an accidental release of a hydrocarbon mixture containing hydrogen sulfide (H2S) during pressure gauge replacement, due to the piping not being effectively isolated. An internal valve obstruction prevented full isolation, leading to the sudden release of toxic gas once the gauge was removed.
